PROJECT ELF - APPLICATIONS OPEN
Applications for Project Elf will open on October 15, 2026 and close November 15, 2026.
YOU MUST SUBMIT THE OFFICIAL FORM TO BE CONSIDERED.
DO NOT SUBMIT THE OLD FORMS OR THE TRADITIONAL HELP FORM. IT MUST SAY PROJECT ELF OR IT WON'T BE CONSIDERED!
No request will be considered that is not submitted on the official Project Elf Application. You have to download the Word document below to fill it out, then save it on your own computer, then you will attach the completed form to an email it to submit. Follow the below instructions.
HOW TO USE THE FORM TO APPLY

The official request form will appear below when applications open. You must click on the form below that works for you (individual or family request form below (individual or family request form).

When it opens, save it on your desktop of your computer as your own file.

Complete the form, save it again.

Email the completed form as an attachment to Info@TheDaneFoundation.org.
Those selected to receive services for Christmas from The Dane Foundation will be notified via email OR text with a pickup date if you are accepted. Typically no less than 10 days before Christmas. Recipients will be required to pick up the donated Christmas items/gifts on a determined date at The Dane Foundation offices in Cuyahoga Falls, Ohio.
No gifts will be delivered by the organization.
Please DO NOT request cash, bill or mortgage payments, expensive electronics like XBox or Cell Phone, etc.
Please note: To qualify for assistance from The Dane Foundation, an individual or member of the family must have a qualifying physical or developmental disability. Examples of these are mental retardation, traumatic brain injury, downs syndrome, cerebral palsy, spinal cord injury, parapelgia, muscular dystrophy, multiple sclerosis, etc.
We DO NOT serve individuals with mental health related disabilities or addictions. Please visit the ADM Board's website or call 211 for a list of providers that serve the mental health and/or addiction populations.
